Administrative Services

How to Register

New patients are welcome to register with us.

New Patient Pack

You will require a New Patient Pack for each person registering with the practice.

Please call the surgery on 0844 477 3543 and ask for New Patient Packs to be sent to you.

Or you can call in at any of our surgeries and request a pack.

You can email the practice to request packs by clicking on the 'Ask a Question' button at the bottom of the page.

Change Your Details

Please see our Online Services, or fill out our Contact Preferences Form to change how we contact you.

Change of Address

If you change name, address or telephone number, please let our receptionists know by calling or writing to us. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.

You can also inform us of changes to your address and telephone number using the internet. For details on how to register for internet access please see Online Services.

Sickness Certificates

Requests for sickness certificates require 48 hours notice. Please hand in your request at reception.

You can also request a sickness certificate by telephone, please call 0844 477 3543.

Test Results

All test results are seen by the doctor that ordered them, or one of the other doctors as needed. Patients are asked to contact the surgery after 2pm to find out their test result. If the doctor needs to see you, we will, of course, contact you.

Patient Transport

This can be ordered via reception. Please give at least five working days notice. Whilst every effort will be made on your behalf, we cannot be held responsible if your transport is late arriving.

Insurance Reports

The GPs will prepare and carry out insurance medicals on your behalf.

Medical Examination

Medical examinations for insurance and other services are regularly undertaken, including HGV, PSV, Taxi driver etc. Details are available at reception.
